I have a Phantom 3 STD and literally shutdown and dropped down from 35 feet to the concrete below.  Props were not turning as it fell so I assume it lost communication with my device (mini iPad).  The DJI GO app was accidentally closed, which is when the aircraft seems to stop functioning.  My question;  is the DJI GO APP required for a controlled flight. ?  If true that would also mean you can not fly craft with RC and craft alone, in any mode ?

It's available to fly aircraft with only RC.
I'd suggest you to sync your flight records from the DJI Go app and get in touch with our tech support at support.us@dji.com.
They will guide you to send it in and analyze the flight data for you.

Even is the DJI Go App is stopped mid flight ?  Will RC still control aircraft ?

The RC cannot control aircraft if the RC signal lost, otherwise RC will still control aircraft.

Thanks - so is the DJI Go App required to fly, or can I fly without device (iPad)
