The flight data doesn't point to an obvious cause but it shows what the problem is and helps eliminate several things that have been guessed at.
The key points I note are that the P4 climbed past the max altitude set at 7:51 while in a waypoint mission.
It recognised the altitude limit but kept climbing.
AT 8:08 the mode switched from the waypoint mission to P-GPS and it under normal circumstances it should have been possible to bring it back from there.
A complicating factor would have been that the P4 was almost straight up more than 500 ft above home point so it would have been difficult to maintain good signal at this angle - but if signal was broken, the Phantom should have hovered in place and not continued to climb.
However it kept climbing until 8:20 @583 feet.
The flight record ends 3.6 seconds later with the Phantom having descended under power at 3.5 m/sec.
I can't tell what commands were being given and how the Phantom responded (or not) but comparison with the Go app playback would show this.
This all looks suspicious enough that I believe it's a case for DJI to deal with.
I don't see signs of any obvious operator error in the material available.
DJI will be able to access the "black box recorder" in the Phantom and see more than is visible to us.
