That is a bad ass tilt shift.
Actually I read about this specific project. Took very very long time. Lots of editing in AE

For the picture it is just a simple effect. Depending on the software you use it is a one click effect (Instagram etc.), or totally manual (photoshop etc)
Just create a second layer with blur effect and then mask some horizontal part out.
Many tutorials online.
Just Google : miniature effect photos or tilt shift effect.

The video is a whole different story. That required some heavy post processing in After effects. And a ton of time.
