I just purchased a Phantom 3 Standard from a friend.  The craft was flying fine when he demo'd it to me.  However, now the signal is weak, telemetry is sketchy, and the video is pixelated or non-existent.

The firmware is the latest version.
I am using a Pixel 3
Tested in differing areas with same results
Max distance achieved around 300 feet.
Had to reset the WiFi password on the unit.

Got any ideas what could be going wrong here?

t could just be where you are flying, some places have more interference than others.

Make sure your antenna is pointed correctly.

No, the drone has not been crashed.

You could always take it back to your friend and get advice on what is going wrong.  Maybe your friend can get it running fine again.  Taking it back to where it last worked would be my first step.

Have you flown in any wet or dirty environments?

Figured it out.  Drone is working fine now.  All it took was re-linking the controller to the drone.
