Software needing updating will not be the cause of the incident.

No-one can tell you how to prevent something unless they know what needs to be prevented.
You will have to post flight data for anyone to be able to tell what your problem was.

It sounds like it might have been a yaw error situation so it would be helpful if you can describe the launch spot.
What was the surface that you launched from?
