I am getting the following below when I try and fill out the online form form for DJI refresh. It happens at the payment screen.

Thanks for the reply. I tried on multiple devices and multiple browsers on each device. Still the same issue. Issue happens everywhere.

Sorry about this. Could you please provide the serial number of your drone, your DJI account, a screen recording of the operation while filling out the information to us for further confirmation?
