I've had the same issue. Most of the time, sitting it on its tripod, on a level surface, and calibrating it will work.If not, try using the hrizonal gimbal agjustment in the DJI Mimo app.
In the Mimo app. If you don't have the app, look for it in the app store under DJI Mimo.
It's in the camera utility section.
You have to make sure it's on a level surface, on a tripod, or the little tripod attachment.
Also, this problem was taken care of in an old firmware update, so, you might need to update the firmware as well.

You can go to Lower left side of the main camera view 3 dots (General Settings) > Gimbal Settings > Gimbal Auto calibration and check if the issue persists. Please keep us posted. I see calibration issues since a couple of days! I need to put the clamp over the buttons in order to have the 12 Pro Max leveled.
Do you also see this issue? Where do you place that clamp? Right over the Apple logo (hence in the center of the phone)?
My clamp is right next to the second button on the left, and works fine.
I'm looking into what I saw in a video about using the magnetic link on the back of the phone.
I've ordered it, and will post just how well it works.

The OM4 is an awesome device, but it's like all devices, they can be frustrating, LOL!

Make sure the magnetic clip is over the apple logo so the phone is not mis-balamced and perform a calibration, that should cure your issues, if it doesn't then come back here and we'll have a further look.

It is pretty mich over the Apple logo :/
When doing some manual adjustments, it works for a while but sometimes it is mis-leveled again!
