If you watch carefully when you hit post you can briefly see tags added to the link before the page submits.  I suspect the JS sometimes does run fast enough before the submit occurs.

Yeah, the media tags are indeed added.
I get those when I'm too quick.

The other thing I’ve seen is when pasting it does come over a clean text, sometimes has a slightly different font and then it’s not recognised.

Ah yes.
I have seen that too.
They probably add some css / shortcode to make these font things work and the js probably ignores that.

It’s usually a bug.  There’s a good one with excel where data pasted in/out can pickup hidden special characters. It’s been a long running issue especially if you paste into things like mainframe (3270) sessions that really don’t know how to handle it.

I’ve not tried it in office 2021 to see if it’s fixed. I need to do a clean install on the laptop for that to avoid the secure boot issue I have with windows 11.
