That seems like the kind of thing that would be mentioned in the manual.
Failsafe RTH begins at the bottom of p16 here:

Hi there,
thank you for reaching out.

The action the aircraft performs once it loses the remote controller signal can be set as Return to Home, Land, or Hover in DJI Fly. If the action was set as Return to Home in advance, and where the Home Point has been recorded, the GNSS signal is good, and the compass is functioning normally, Failsafe RTH will automatically activate once the remote controller signal is lost for more than three seconds.

Lots of people have tested it and you can too.
Launch from a safe area with no obstacles close by, fly out 100 feet or so, switch off the controller and watch it happen.

And I know this will work, I tested this years ago with the Phantom 1 & 2 class of drones way back when,   I was just surprised when the mini 3 did not rise to the RTH height after a good 10 seconds of signal loss.

If it had I feel sure I could have reconnected, but as it was had to go walk abouts to get a better signal,  I am interested if anyone else might have had this experience as well.

My working theory is that I did not have a full signal loss, and more a long intermittent breakage, but not reconnecting long enough for me to regain control, but enough for RTH to not kick in.

I think this is where experience plays a part, as anyone can read the manual and assume its gospel,  in in practice it might not always work that way.

Has anyone else suffered a RTH failure like this ?

It's possible to only lose the downlink but still have the uplink.
In that situation the drone won't RTH, but if you switch off the controller, you would lose all signal and trigger Failsafe RTH.

If you had only lost the downlink, the recorded flight data from that incident would confirm that.

Fair enough, that sounds plausible.  The drone did not respond to inputs though to be fair.
