I received a DJI Care Refresh case number via telephone from customer support and was told to expect an email
containing shipping instructions and a shipping label.

The email has never arrived.

Where do I ship to ?

It can take 24-48 hours for a shipping label and RMA information to be generated. Since you generated the case number earlier this morning, you will be receiving everything you need shortly.
