P4 Video Transmission Problems Resolved
I downgraded the DJI Go app to version 3.0.1 Problem Fixed.

I have installed all the latest software updates and tested this problem again on DJI GO 4.0 app. I am currently using
App Version for Android 4.0.0
App Version for iPhone 3.1.1
Aircraft Version 01.02.0602
Remote Controller Version 1.8.0
HDMI module installed
Wifi and bluetooth are turned off
Cache locally when recording is turned off
The video is still pixelated in green and black. The good news is that now the video is working on iPhone 6S and through HDMI. However, on all the Android devices I'm using (Sony Xperia ZX phone, Z2 phone and Xperia tablet) the video is not working. On iPhone, the horizon keeps tilted even after calibration.
