Calibrated, then took out for a quick test flight. Hovered in front of me for a couple minutes without issue. Then I sent it out a bit over the ice. Got an "idle motor" error message and it started the death spiral. Threw a prop in the process.
I was really sure the props were all put on properly. Maybe that one wasn't.
Either way, it came out fine. At least it crashed with the gimbal facing up!
I let it thaw out and dry up. Flew it again a few hours later. No issues since.
I had the same experience with my first RMA. Got about 30 feet in the air and WHAM...death spiral. I called DJI and they sent me another. My rear left motor had a bad prop lock causing the prop to fly off after about a minute.

I'd say that's what happened in my case. I have two Mavics, fortunately, so it's not like I'd likely make a huge mistake putting the prop on.
