I sent my crashed Mavic Air off to DJI for repair under warranty, which they graciously did. Upon return, it appears that they actually sent me a new drone instead of sending back the old one repaired. And so it is asking me afresh, "You still have 48 hours to purchase DJI Care Refresh". But in fact my existing Care Refresh contract still applies, right? Because it applied to the drone I had, which they opted to replace.

I would think your DJI Care Refresh transfers no matter whether the repair was counted under your two replacement count per year, or a straight warranty claim replacement. Either way, you should consult with DJI to confirm your Care Refresh is in place under the new drone.
