There has been notification from DJI in the forum, of this issue in some Phantom 3 models.
If you do a search you can find the information. Your Phantom 3 may have to be returned, for repair.

Have you tried a second SD card, to see, if by a small chance, it's the faulty card and not the Phantom card reader?

Did you buy your Phantom 3 kit locally at a retail location, or from an online vendor...?
If local, perhaps return it for an exchange.
It won't work the way it is, so it needs to be fixed, or replaced.

You might try contacting the seller, asking them first, and see if they will replace it for you.
Otherwise, it may have to be sent to DJI for assessment and repair or exchange.

Make sure you are not trying to insert a card on top of one that is already in the slot.

This is an older thread, so hopefully the OP has been cleared for takeoff.  His was definitely not from a second card introduction.
That would be too easy, to figure out.

In this subsequent case, can the poster please upload a short video or couple of closeup pics, so we can alteast, try to figure out, a quick fix.

The myriad of failures can often be attributed to poor final check, on the factory floor.
Hopefully this can be turned around. ;-)
I feel bad for all those, who are experiencing these devastating failures.
At these relative high prices for a quality piece of merchandise, it would seem even more painful for them, to me.
