It happens on both my Xperia Z3 and Z4 Tablet. I haven't tried on ipad yet.
All firmware and app are up to date. USB cable is a good (Anker 1ft) one.

I FOUND MY SOLUTION!!!!  Even though the DJI website says you can operate the Phantom 4 with a iPad Mini 2, I have come to the conclusion that the processor is too slow.  I have installed an upgraded antenna on the controller first (no change) and have upgraded my operating platform to an iPad Mini 4 (much faster processor... and at a reduced price as of this week.)  No more green and black bars while flying.  YAHOO!!!!

Hello ngvuanh, the original videos in SD card is perfect, right? would you please clarify the firmware version for aircraft, remote and APP? all are up to date?
If it's only occurs for image transmission. Please cool down your mobile device, turn off all background programs before flying.
Just as Air54 mentioned, you could also try to upgrade operating platform.
Besides, please check the channels on HD page through APP, and you could customize it and decrease quality of image transmission, and fly in an open place without interference.
