This is for I followed the the update instructions using the SD card method.
The update ran as described for a long time with gimbal movment flashing lights of all colors and gimbal light flashing green and red.  Then rear lights went to blinking yellow, gimbal light went solid red and a solid audio tone.  I let it stay in this state for about 20 minutes not sure if the update was finished.  After looking at some forumes this was a sign of a failed upgrade.
So I powered off checked the logs and sure enogh it idicated failed.  Logs below.

Result: Failed.
Reboot the aircraft and try to upgrade the firmware again.

Now every attempt I do to rerun the upgrade it does not go through the 25 minute upgrade process.  It starts the upgrade and within 45 seconds goes straight to the  rear lights went to blinking yellow, gimabl light went solid red and a solid audio tone.  What are next steps anyone know?

Geoff, There's a detailed upgrade log that is hidden in the MISC\LOG folder (again, it is hidden and you must enable viewing hidden files to see it.) It wil give additional information about the specific module(s) tat s failing. Take a look and post.

Device [15 00] is the Camera Air Encoder; device [08 00] is the Air End USB and the upgrade didn't see it from the initial upgrade onward. You do have everything else shutdown (RC, Tablet/Smartphone) correct?

Correct nothing was connected or powered on during the upgrade.  
Could it be a loose cable?  I looked at all the external cable connections and they look solid.

In previous upgrades, turning off the P3 and allowing it to cool down, formatting the SD card on the P3 and starting again from scratch sometimes worked.

Oh no.  So what you are saying is the camera very well maybe defective and I have to get it repaired or a a new aircraft via warranty?  This aircraft is just 3 weeks old and has not even had a hard landing.  It has been operating perfectly for the 3 weeks I've owned it.  What is best course of action, new aircraft via warranty or attempt repair?  I'm new to DJI so any advice would be much appreciated.

Try formatting the SD Card on the P3, vice a workstation/laptop. And grab a fresh copy of the download just in case... Then try one more update...

Ok great news following advanced troubleshooting did the trick for the aircraft.  It seemed that removing the battery for 10 minutes and then the upgrade ran as expected!!!  Thanks TerryLewis and Tahoe!!!

Now the controller firmware will not upgrade.  I never see the blue light it just goes from red to green.  If I open the app it says the controller needs to be upgraded.  BUt if I look at the output of the log it states the firmware on the card is up to date or older so aborting the attempt???  So is the controller up to date or not updating at all?

   The latest firmware does not update the RC. So the most current RC firmware is 1.3.20.

I have had trouble trying to upgrade to 1.04. First attempt there were all sorts of flashing lights going on and the beeps were like an echo. Left it for about 35 minutes then turned it off. Tried again after everything had cooled down. Re installed the firmware onto the card and turned it on. No update beeps of any kind. Left it again for some time and nothing. Formatted the card, reinstalled the firmware. Same again no update beeps. Put the card into the PC and it had installed the DCIM folder, the Misc folder plus a log and a Txt folder. The log folder only showed the update 1.03 that I installed about 5 weeks ago. No mention of 1.04 at all. Tried 2 other SD cards and the results were the same, no mention of 1.04 except on the extracted firmware. How do I read the hidden log files that I have opened

Rizal do you finally solve your problem? I have the same issue, even after upgraging to firmware 1.5 still doesn't work and had the no signal issue (blank screen).

You have the correct firmware for your Phantom3 Pro, can you please reformat the SD card and download the file again.
The process takes up to 25 minutes or so, when you power up the Phantom with the freshly downloaded firmware let it sit for 25 minutes. If it doesn't update , then remove the SD card and copy the log and post it again.
